RELEASE OF LIABILITY
I am voluntary attending the Carolina Christian Students to Black Mountain, NC on Nov 4, 2023. While participating, I agree to hold free and harmless and fully and completely release Carolina Christian Students, the officers of Carolina Christian Students, and any members/participants of the student organization, the Church in Columbia, and the University of South Carolina from any and all liability, claims, and/or demands for personal injury of myself, as well as, but not limited to damage and expenses of any nature whatsoever that I may incur while participating and traveling to and from stated event.

MEDICAL AUTHORIZATION
In case of an emergency, I give permission to receive first aid, trauma care, and/or to be taken to a medical facility by any staff member of Carolina Christian Students. I authorize non-emergency and emergency medical treatment and assume the responsibility of any medical bills and expenses accrued while at the event and in transport to any medical facility.
